Promiz Injection 40mg is a high-potency parenteral Proton Pump Inhibitor (PPI) formulation containing Omeprazole Sodium (40mg per vial). Engineered for immediate systemic bioavailability via Intravenous (IV) injection or infusion, Promiz Injection delivers rapid and sustained inhibition of gastric acid secretion. It is indicated for the acute management of Gastroesophageal Reflux Disease (GERD) with esophagitis, gastric and duodenal ulcers when oral therapy is inappropriate or non-feasible, stress ulcer prophylaxis in critically ill patients, and the prevention of rebleeding following endoscopic treatment of acute peptic ulcer hemorrhages.
\n \nRapid & Potent Acid Suppression: Achieves immediate systemic action to elevate intragastric pH in acute and critical clinical settings.
\nEffective Peptic Ulcer Hemostasis: Maintains high intragastric pH () to promote blood clot stabilization and lower the risk of recurrent peptic ulcer rebleeding.
\nEssential Inpatient Alternative: Serves as a reliable parenteral option for surgical, unconscious, NPO (nothing by mouth), or severely dysphagic patients.
\nICU Stress Ulcer Prophylaxis: Protects critically ill patients from acute upper gastrointestinal mucosal lesions and bleeding.
\nTargeted 24-Hour Control: Sustains elevated intragastric pH levels to facilitate rapid esophageal and mucosal ulcer healing.
\nPromiz Injection 40mg (Omeprazole Sodium) acts as a targeted, irreversible inhibitor of the gastric -ATPase enzyme system (the proton pump) located on the secretory membrane of parietal cells. Following IV administration, Omeprazole diffuses into the acidic canaliculi of gastric parietal cells, where it is converted into its active sulfenamide metabolite. This active form binds covalently to the cysteine residues of the -ATPase enzyme, blocking the final step of gastric acid secretion and inhibiting both basal and stimulated acid release regardless of the secretagogue stimulus.
\n \nOmeprazole Sodium (equivalent to 40mg Omeprazole per vial): Intravenous substituted benzimidazole proton pump inhibitor that blocks parietal cell -ATPase to suppress gastric acid release.
\nReconstitution: Reconstitute the 40mg dry powder vial with 10ml of 0.9% Sodium Chloride Injection or 5% Dextrose for slow IV injection, or dilute further in 100ml of compatible IV solution for IV infusion.
\nAdministration:
\n \nIV Injection: Administer the reconstituted solution slowly over a period of at least 2.5 to 5 minutes (maximum rate of 4ml per minute).
\nIV Infusion: Infuse diluted solution over 20 to 30 minutes.
\nPeptic Ulcer Bleeding Protocol: Typically administered as an initial 80mg bolus infusion followed by a continuous infusion or intermittent 40mg doses as directed by the attending gastroenterologist/intensivist.
\nProfessional Healthcare Use: Must be administered exclusively by a qualified healthcare professional in a clinic, hospital, or critical care setting.
\nPrescription-Only Parenteral: Strictly for professional healthcare administration; do not self-administer.
\nAvoid rapid IV push; ensure administration speed strictly follows clinical guidelines to prevent local vascular irritation.
\nDiagnostic evaluation is recommended prior to initiation to rule out gastric malignancy, as PPI therapy can mask warning symptoms (e.g., unintended weight loss, persistent vomiting, dysphagia, hematemesis).
\nAdjust dosage in patients with severe hepatic impairment (maximum recommended daily IV dose is 20mg).
\nConsult an attending physician prior to use during pregnancy or lactation.
\nKeep out of reach of children.
\nStore the un-reconstituted dry powder vial in a cool, dry place below 30°C, protected from direct light and heat. Reconstituted solutions should be used immediately or within 4 hours if kept below 25°C. Protect reconstituted solution from light. Do not freeze.
\n \nFormulated and manufactured in strict compliance with international pharmaceutical quality standards, dedicated to engineering advanced gastroenterology and critical care parenteral therapies that restore mucosal integrity and improve patient outcomes.
