Oraject-D IM Injection is a specialized dual-action parenteral formulation combining a high-potency corticosteroid (Dexamethasone) with a long-acting local anesthetic (Bupivacaine Hydrochloride or equivalent anesthetic agent). Engineered specifically for Intramuscular (IM) or localized peri-articular/soft tissue infiltration, Oraject-D provides rapid local pain relief alongside sustained anti-inflammatory and immunosuppressive action. It is indicated for the targeted management of severe acute musculoskeletal pain, joint inflammation, radiculopathy, bursitis, tendinitis, and localized post-traumatic or post-surgical inflammatory conditions.
\n \nDual-Action Pain & Inflammation Relief: Combines immediate local nerve blockade for rapid analgesia with sustained corticosteroid activity to halt inflammatory cascades.
\nTargeted Soft Tissue & Joint Relief: Provides focused therapeutic action directly at localized sites of inflammation, bursitis, or muscle spasm.
\nProlonged Analgesic Duration: Long-acting local anesthetic component offers extended pain relief, reducing the need for systemic opioid analgesics.
\nPotent Anti-Inflammatory Efficacy: Dexamethasone rapidly suppresses tissue edema, vascular permeability, and local pro-inflammatory cytokine release.
\nParenteral Convenience: Formulated for precise intramuscular or peri-articular clinical administration.
\nOraject-D IM Injection operates via two complementary physiological mechanisms:
\n \nDexamethasone (Corticosteroid): Diffuses into target cells and binds to glucocorticoid receptors, inhibiting the transcription of pro-inflammatory cytokines and enzymes (such as cyclooxygenase-2 and inducible nitric oxide synthase). It stimulates lipocortin synthesis, blocking phospholipase and preventing the downstream release of arachidonic acid derivatives (prostaglandins and leukotrienes).
\nLocal Anesthetic Component: Voltage-gated sodium channel blocker that reversibly stabilizes neuronal membranes. By inhibiting sodium ion influx across nerve axons, it prevents action potential generation and propagation, blocking pain signal transmission from the inflamed site to the central nervous system.
\nDexamethasone Sodium Phosphate: Synthetic glucocorticoid providing powerful local and systemic anti-inflammatory action.
\nLocal Anesthetic Agent (e.g., Bupivacaine / Lidocaine HCl): Long-acting sodium channel blocker providing local analgesia and nerve conduction block.
\nAdministration Route: Strictly for Intramuscular (IM) injection or localized peri-articular/soft tissue infiltration. Do NOT administer via intravascular (IV) injection.
\nAdministration Method: Administer via deep intramuscular injection into a large muscle mass (e.g., gluteal region) or direct local infiltration at the target inflammatory site under aseptic clinical conditions.
\nDosing: Dosing and frequency are determined strictly by the attending clinician based on the specific indication, site of inflammation, and patient weight.
\nProfessional Healthcare Use: Must be administered exclusively by qualified medical personnel in a clinic, hospital, or pain management facility.
\nPrescription-Only Parenteral: Strictly for professional healthcare administration; avoid self-administration.
\nAccidental Intravascular Injection: Ensure careful aspiration prior to injection to avoid accidental intravascular delivery, which can lead to cardiovascular collapse or central nervous system toxicity.
\nContraindications: Do not administer into infected sites, active systemic fungal infections, or in patients with known hypersensitivity to corticosteroids or local anesthetics.
\nAdrenal Axis Suppression: Prolonged or repeated systemic exposure may suppress the hypothalamic-pituitary-adrenal (HPA) axis; dosage should be tapered appropriately if used over extended periods.
\nConsult an attending physician prior to use during pregnancy or lactation.
\nKeep out of reach of children.
\nStore in a cool, dry place below 30°C, protected from direct sunlight and heat. Do not freeze. Retain in original protective outer carton until time of administration.
\n \nFormulated and manufactured in strict compliance with international pharmaceutical quality standards, dedicated to engineering specialized parenteral formulations for effective pain management, rheumatology, and clinical emergency care.
